Liaison & Consultation

Liaison and consultation incorporates:

• Multidisciplinary team participation in case conferencing and care delivery.
• Contributing to, or participating in the Medication Advisory Committee.

 

Knowledge

Demonstrate understanding of:

• the role of the health professional team in the delivery of care
• the contribution of health care teams in the delivery of high quality care, and
• the role of peak bodies as sources of information and guidance.

Skills

Demonstrate ability to:

• communicate with a variety of health professional practitioners to support clinical decision making
• act as a nexus for communication between members of the team, the resident and families, and
• coach other staff and facilitate communication to improve care delivery.

Tasks

Include a range of health professionals in care planning and delivery.
